Advance Planning
If you would like to plan your funeral or memorial service in advance, you can use the form to specify your preferences:. Funeral or Memorial Service Planning Online Form (PDF Version of Funeral Planning Form). It can be very helpful to family and friends if you make your wishes known about the type of service elements you'd like to include in your memorial service or funeral. You can also designate plans here for Plymouth's Memorial Garden (available for members) and memorial gifts. If you fill out the online form, we will be notified. If you download and print the PDF form, please drop it off or mail it to the church once completed. Our clergy are pleased to discuss plans with you, and can be reached at 970-482-9212 or by email.
