If you fill out the online form, we will be notified. If you download and print the PDF form, please drop it off or mail it to the church once completed.
- Fundraising Request Form Note: No special fundraising will be approved for September-October, as it conflicts with our annual pledge drive.
- New Member Form ("pink sheet"), submitted at the end of a New Member class.
- New Ministry Team Covenant Form (downloads PDF) Please print the form, and drop it off at church or mail it to the corresponding board chair or to a minister.

- Volunteer ("Safe Church") Form (downloads PDF) Preventing abuse and harassment is critical in the life of our congregation. All persons wishing to volunteer will fill out the form attached to our policy. Please print out the PDF file, complete it, and return it to our church office before working with children, youth, or vulnerable adults. Questions?
- Annual Report Form to submit an annual report to the office for the January Annual Meeting. Due 12/31. Please compose your report in whatever software you normally use, then copy and paste into the form.
